The concepts describing how care activities form processes, and the requests and referrals that drive them.
- care process
  set of interrelated or interacting care activities which transforms inputs into outputs
- continuity of care process
  care process, encompassing all care provider activities and other prescribed care activities, that addresses identified or specified health issues
- input health state
  health state at the start of a care process
- output health state
  health state at the end of a care process
- request for care
  request for care provider activities
- initial request for care
  first request for care concerning one or more specific health matters to be assessed by a care provider
- referral
  request for care where a care professional asks a care provider to make a care commitment for a care period mandate
- request for service
  request for care where a care professional asks a care provider to deliver one or more care service offerings

  care evaluation where a care process is evaluated against requirements

Example 1 Request for a care assessment, an operation, a wheelchair.

Example 2 Request for a second opinion where the responsibility remains with the requesting party.

Note 1 The responsibility for the requested care provider activities is held by the performer but they will be performed under the care period mandate of the requester.

Note 2 A care provider may accept or decline a request for service.
