The concepts describing how care activities form processes, and the requests and referrals that drive them.
- care process
  set of interrelated or interacting care activities which transforms inputs into outputs
- continuity of care process
  care process, encompassing all care provider activities and other prescribed care activities, that addresses identified or specified health issues
- input health state
  health state at the start of a care process
- output health state
  health state at the end of a care process
- request for care
  request for care provider activities
- initial request for care
  first request for care concerning one or more specific health matters to be assessed by a care provider
- referral
  request for care where a care professional asks a care provider to make a care commitment for a care period mandate
- request for service
  request for care where a care professional asks a care provider to deliver one or more care service offerings
- reason for request for care
  motive for a request for care
- care process evaluation
  care evaluation where a care process is evaluated against requirements

These concepts describe the care process and its transformation of an input health state into an output health state, together with the requests for care and referrals that drive it.
