Example 1 To increase the subject of care's adherence to the treatment in a hypertension programme.
Example 2 To stabilise the subject of care’s systolic and diastolic pressure in a hypertension programme.
Example 3 To increase the subject of care’s knowledge of their disease through an educational programme for diabetics.
Note 1 A set of care goals may constitute an intended outcome.
