The concepts describing how care planning produces the family of care plans — guided by guidelines, protocols, and pathways — and the unintended events plans must anticipate.

- care guideline

Plural care pathways

Note 1 The concept care pathway includes subtypes, for example, “integrated care pathways”, “multidisciplinary pathways of care collaborative care pathways”.

Note 2 Care pathways are designed to support care activity management. They provide detailed guidance for each stage in the management of a subject of care (treatments, interventions, etc.).

Note 3 Care pathways are knowledge supporting the design of core care plans.
