Health matters

The concepts describing how a person is: their health state, the clinical and social matters it comprises, and the health conditions identified within it.

These concepts describe how a health state frames a care process from input to output health state, the kinds of health matter — clinical and social — that care addresses, and how health conditions are identified, observed, considered, and professionally assessed.
